Survey Field Types

The Gravity Forms Survey Add-On makes it quick and easy to create a survey. Once you’ve installed the add-on, you will find a Survey field in the form editor – within this Survey field, there are eight Survey field types from which you can create your questions.

• Likert Scale – The user can choose an answer from a scale of options.
• Rank – Users can drag and drop options to rank them in their preferred order.
• Rating – Users can select a star rating.
• Checkboxes – Users can pick multiple options.
